By completing and returning this form I agree:

• For the STOW Implementation Board to act on my behalf to conciliate my complaint.
• That the personal information contained in this form and any supporting evidence can be shared with the person/ company stated on this form (respondent).
• That the person/ company stated on this form can provide the STOW Implementation Board with evidence relating to my complaint, including, but not limited to, documentation, emails, call recordings and CCTV footage.

I understand that if I choose not to provide the information requested, or ask for it to be deleted, the STOW Implementation Board may not be able to conciliate my complaint. I understand that the sharing of this information with the person/ company stated on this form may require data transfer within Trinidad & Tobago. I understand that the data will be stored at the Energy Chamber of Trinidad & Tobago.

I would like the STOW Project Office to consider my complaint.
